One Call Lab Collection provides mobile blood draw and lab collection services ("Services"). By scheduling and using our Services, you agree to the terms outlined in this Agreement.

2. Appointment Scheduling:

  • Clients are responsible for scheduling their appointments through our designated booking system or customer service.

  • We make reasonable efforts to accommodate your preferred appointment time, but appointments are subject to availability.

  • You must provide accurate and complete information when booking your appointment.

3. Fees and Payment:

  • All fees for services will be communicated to the client prior to the appointment.

  • Payment for services is due at the time of service unless otherwise agreed.

  • We accept payments through various payment methods, including credit card, debit card, and other payment processors.

  • Late payments or failure to pay may result in cancellation of further services or the application of late fees.

4. Client Responsibilities:

  • You agree to provide accurate and complete information regarding your medical history, prescriptions, and health conditions to ensure the safety and efficacy of our services.

  • You must ensure you are available at the agreed-upon time and location for your appointment. A failure to be present or notify us of any change in your availability may result in a cancellation fee or rescheduling charges.

  • You are responsible for providing any necessary materials or samples for lab processing.

5. Privacy and Confidentiality:

  • We take your privacy seriously. All personal information, including health-related data, will be collected, stored, and processed in accordance with our Privacy Policy.

  • We are committed to maintaining the confidentiality of your personal and medical information in compliance with applicable privacy laws (such as HIPAA).

6. Health and Safety:

  • Our trained professionals will perform all services in a safe, professional, and hygienic manner.

  • You agree to follow any instructions provided by our staff during the collection process.

  • In case of an emergency or if you have any questions during or after the service, you agree to immediately contact our customer support.

7. Cancellations and Rescheduling:

  • Clients may cancel or reschedule appointments up to 24 hours in advance without penalty.

  • Cancellations made less than 24 hours before the appointment may incur a cancellation fee.

  • We reserve the right to cancel or reschedule appointments due to unforeseen circumstances, such as inclement weather or staffing issues. In such cases, we will notify you promptly and reschedule at your convenience.

8. Limitation of Liability:

  • While we strive to provide high-quality services, One Call Lab Collection is not responsible for any issues arising from incorrect or incomplete medical information provided by the client.

  • We will not be liable for any direct, indirect, incidental, or consequential damages, including those related to delays, missed appointments, or errors in the lab results.

9. Modifications to Services:

  • We reserve the right to modify, suspend, or discontinue any part of our services at any time, with or without notice. In case of significant changes, we will notify clients as appropriate.

10. Compliance with Laws:

  • You agree to comply with all applicable local, state, and federal laws regarding the use of our services, including any health-related regulations such as HIPAA.

  • You are responsible for ensuring that any medical procedures or tests performed by our services are in accordance with your physician’s advice and relevant medical guidelines.

11. Intellectual Property:

  • All content provided by One Call Lab Collection, including but not limited to logos, designs, trademarks, and written materials, is owned by us and is protected by intellectual property laws. You may not use or reproduce any of this content without our express permission.

12. Dispute Resolution:

  • In the event of a dispute, we encourage you to contact us directly so we can attempt to resolve the matter amicably.
